End User License Agreement

This End User License Agreement (“Agreement”) governs your use of the DPerspective chronic-disease management application and its backend services (the “Service”).

1. License Grant

DPerspective grants you a personal, non-exclusive, non-transferable, revocable license to use the Service for managing your own health information. You may not sublicense, resell, or redistribute the Service.

2. Acceptable Use

You agree to use the Service only for lawful purposes and only to manage health information that belongs to you or that you are authorized to manage. You must not attempt to access another user’s data, circumvent the Service’s security controls, or interfere with its operation.

3. Health Information

The Service stores protected health information (PHI) that you provide. DPerspective is a self-management tool and does not provide medical advice, diagnosis, or treatment. Always consult a qualified healthcare professional.

4. Termination

This license terminates automatically if you breach this Agreement. On termination you may request export and erasure of your data as described in the Privacy Policy.

5. Disclaimer of Warranties

The Service is provided “as is” without warranties of any kind to the maximum extent permitted by law.

6. Limitation of Liability

To the maximum extent permitted by law, DPerspective is not liable for indirect, incidental, or consequential damages arising from your use of the Service.
